Does Trip Decision Review include booking or provider contact?

No. Ticked Bucket List may give you questions to ask and explain why the answers matter. You contact providers and make the bookings.

Can the $69 Starter Kit price be applied to the review?

If you bought the Starter Kit for the same trip within the previous 90 days and submit a usable Trip Snapshot, the $69 may be applied to the review. This must be confirmed before the review invoice is issued.
